Endocrinologist in Latviawhere to get treatment, how to verify certification, what this doctor does

An endocrinologist specialises in the prevention, diagnosis and treatment of disorders of the endocrine glands. They read hormone levels alongside symptoms, adjust medication doses, and follow patients with chronic endocrine disease over the long term.

• Diabetes: type 1, type 2, gestational and other types
• Thyroid disease: thyrotoxicosis, hypothyroidism, thyroid nodules
• Calcium metabolism disorders, parathyroid disease, osteoporosis
• Sex hormone disorders: hypogonadism, polycystic ovary syndrome, hirsutism, fertility problems
• Adrenal disease: adrenal insufficiency, adrenal masses
• Pituitary disorders: hypopituitarism, acromegaly, hyperprolactinaemia

A family doctor is enough while a chronic condition is stable and blood results are good. An endocrinologist is needed when the diagnosis is unclear, when treatment is not working, or when insulin therapy has to be started and titrated.

Endocrinology in Latvia has no separate sub-specialties. In practice doctors often concentrate on one area, such as diabetes care, thyroid disease or osteoporosis, and bone metabolism has its own separate association. Paediatric endocrinology is a separate specialty with its own page.

• Six years of medical study and a doctor's degree, at RSU or the University of Latvia.
• Residency in endocrinology. RSU states that three years cover the knowledge and practical skills required under Cabinet Regulation No. 268 and European Union recommendations. Programme page: RSU residency programme.
• During training the resident works in a specialist endocrinology ward or endocrinology consulting room under a supervising doctor.
• Each year the resident submits at least four clinical cases to the head of the RSU Department of Internal Diseases and takes part in teaching students.
• The certification and recertification requirements for the endocrinology certificate are drawn up and updated by the Latvian Association of Endocrinologists.
• The certificate is valid for 5 years. Recertification requires 250 continuing education points, at least 60 per cent tied to the base specialty, plus a 48.38 EUR fee.
• Entry in the practitioners register must be renewed every 5 years.

• Languages. Consultations run in Latvian and Russian, with some doctors also working in English.
• Area of focus. Doctors often concentrate on diabetes, thyroid disease or bone metabolism, and the clinic profile usually says which.
• Diabetes education. If insulin therapy has just started, a provider with a diabetes education room is worth choosing.
• Tests on site. Some endocrinologists perform thyroid ultrasound themselves; elsewhere it is a separate referral.
• Second opinion. Patients may freely choose the doctor and institution, including asking another specialist to review an existing diagnosis and treatment plan.

• Bring recent blood results, including glucose and HbA1c and thyroid hormones, earlier imaging reports, and a full list of medicines.
• For patients with diabetes, a self-monitoring diary or glucometer data is useful.
• At the visit the doctor assesses general condition, palpates the thyroid, listens to the heart and measures blood pressure, then runs the tests specific to the suspected condition.
• For diabetes the doctor reviews the diary, checks understanding of diet and exercise, and for insulin users also injection technique, referring to further diabetes education where needed.
• Repeat visits are usually needed to track test results and treatment response, and to adjust doses.

People with diabetes can see an endocrinologist without a family doctor referral, since for them the endocrinologist is a direct-access specialist. Everyone else needs a referral for a state-funded consultation, which then costs a 4.00 EUR co-payment. Ultrasound costs 4.00 EUR, and co-payments in one calendar year are capped at 570.00 EUR.

The specialty body is the association "Latvijas Endokrinologu asociācija" (LEA), registration number 50008173791, registered address Pilsoņu iela 13, Rīga, LV-1002, phone +371 67042508, email endokrinologiem@gmail.com, endokrinologi.lv. It was founded on 4 March 2011. Its purpose is to bring together endocrinologists and other doctors and researchers working on endocrine disease and diabetes care, to develop diagnosis and treatment guidelines, and to draw up and regularly update the certification and recertification requirements. Chair of the board: Kristīne Geldnere.

Bone metabolism is represented by a separate association, "Latvijas Osteoporozes un kaulu metabolo slimību asociācija", registration number 40008158790.

Certificates are issued by the Certification Council of the Latvian Medical Association, founded in 1988. The sector regulator is the Health Inspectorate.

If you have doubts about the quality of care, raise them first with the institution's management, which must operate a complaints and suggestions system. If that does not resolve it, a written submission goes to the Health Inspectorate no later than two years after the alleged infringement, and is reviewed within 30 days, extendable to four months in complex cases.
